By clicking the link to access the Sources of Distress Survey, you are leaving the ACU website and navigating to a secure, external platform operated independently by University of Utah Health and the Huntsman Mental Health Institute. The ACU does not host this application, nor do we collect, process, route, view, or store any Protected Health Information (PHI) or personal data submitted through it. The ACU has no access to your responses or survey results.
